We may verify your identity by phone call, text message, email, or third-party verification. Depending on your request, we may ask for information such as your name, email address, phone number, current or former address, or other information related to transactions between you and Chalice Kitchen & Bath. We may also ask you to provide a signed declaration confirming your identity. If we decide not to take action on your request, we will inform you of the reason for our decision. NORTH CAROLINA PRIVACY RIGHTS

North Carolina General Statutes § 75-61, § 75-62, and § 75-63 indicate that organizations should disclose whether certain categories of information are «sold» or transferred for an organization’s «business purpose» as defined under North Carolina law. North Carolina law also requires organizations to provide a notice of financial incentives. We offer an incentive program for PRO customers that provides certain perks, such as bonus incentives and program benefits (the «Pro Premier Rewards Program»). When you sign up for the Pro Premier Rewards Program, we will ask you to provide your name and email address. Because our Pro Premier Rewards Program involves the collection of personal information, it might be interpreted as a «financial incentive» program under North Carolina law. The value of your personal information to us is reasonably related to the value of the free or discounted products or services that you obtain through the Pro Premier Rewards Program, based on the expenses related to offering those benefits.
